Pharmac is seeking feedback on pharmaceutical labelling preferences. This is intended to reduce the risk of medication errors and improve patient safety; align with best practice and ensure consistency.
This is our opportunity to provide feedback on the ways in which labelling of medicines can be improved to help prevent errors. Nurses are the primary administrators of many medicines and are frequently most at risk due to poor labelling or similar labels.
I would be keen to hear of any examples of where labelling confusion, similar labelling or other labelling issues have contributed to errors or near errors. All replies will be treated in confidence.
Any other comments, suggestions or feedback is also very welcome.
